Once you have joined the plan and received your membership kit, simply follow these directions and start using your membership card immediately! Some discounts will have specific directions on how to use. Please review your membership kit for details.

To locate and verify participating providers in your area, simply call Member Services at (888) 866-3326 or use the PROVIDER SEARCH tool. When calling to schedule an appointment, please refer to your membership card for proper network identification.
Step 3
You are responsible for paying the provider the entire discounted fee at the time the service is provided.

Request a video or telephonic consultation with a DialCare physician.
